INTRODUCTION : WHY
COMPUTERSPIROMETRIE ?
Using personal computers for registration and storage
of medical signals like spirometrie
gives enormous advantage.
- Heavy duty , no mechanical parts inside.
- Very simple to use.
- Better patient cooperation. His can follow his
results on the screen.
- The PC filters and put in order the signals
- A4 output on normal paper.
- Build in database, very easy to recall a patient.
- Comparison on screen with a previous record.
- Saving and loading the signal.
- Personal conclusion TOP
Measures are only
15 * 8 * 4.5 cm.
Easy to keep in the hand.
Complete
hygienic :
Disposable mouth-pieces.
The apparatus is connected with a 1.5 meter long cable
to the F12 apparatus.

Technical
data :
- Power consumption only 5 ma.
- Flow resolution : 10 ml / second
- Flow capacity :
15 litre / second
- Sampling
rate 300 / second.
- Registration time 20 seconds or less
One high density 3.5 inch disk or CD-ROM.
The installation is similar to the ECG installation
program
Put the floppy disk drive A: or the CD-ROM in its
drive
Click Start - Run - A:\SETUP.EXE or X:\SPI32\SETUP.EXE
Target directory C:\SPI32. You can give another directory with Browse.
After the installation you find in the directory

What is the function of the option Tracé - Reindex ?
Don't
forget to execute it if you use the Windows Explorer to copy, move or delete
*.spi recordings.

LANGUAGE
: Select your language.
COMPORT
:
Select a free serial port to wich your apparatus is
connected.
If recording works properly, then don’t change this
setting, except if you have moved to another PC.
If you are using a desktop PC, you’ll probably have
to choose COM2.
If you are using a portable PC, you’ll probably have
to choose COM1.
ECGTYPE

Select Ecg apparatus acquired <1/1/2000 or >1/1/2000
If
you select
PRINTER-PRINTER : Select the printer that will
be used for ECG printing.
FONT

Very
important ! If you enter the wrong directory, the program will not
work
properly !!!
Meteorological
data. :
AIR
PRESSION in mm Hg.
TEMPERATURE
in degree Celsius.
This must be entered before the registration.
These parameters can have a light influence on the
measured values.
CORRECTION
FACTOR :
The gain of the amplification of the measured values
can be adjusted with the correction
factor.
You need to verify the measured value on a regular
basis.
The easiest way is to compare the PEF value to the
value measured with a good Peak Flow meter.
If
your PeakFlow is 600 with a meter and you see on the screen PEF 300, than
the
correction factor must be doubled.
Ok
: Settings are saved in C:\WINDOWS\SPI32.INI.
Cancel
: Quit the setup screen
without saving.
Turn the Ecg apparatus ECG F12 ON.
The spirometer must be warmed up to give the best
results.
Place the patient so that he can see the screen of the
monitor.
Click on -
> (1)
Tab key : jump to next field.
Shift – Tab : previous field.
Only
last 4 fields are obliged. Sexe
M of F
Age in years and
Length in cm Weight in kg.
They are necessary for the calculations of the
reference values..
PS
:
The age is not automatically calculated from the birth date.
It is not possible to impose on most users an uniform
way to give the birthdate.
Click Ok to
leave this screen.
Put the mouth-piece to the spirometer.
If the patient has never performed a spirometrie,
it’s better to show it once.
Maybe place a clamp on the patient’s nose.
Click on 1.
Inspire and expire as hard en deep en long as possible.
Repeat this a couple of times.
It is particular important to expire long enough, at least 3 seconds.
Spirometrie demands a good cooperation of the patient.
If results are not ok, start again. No paper has been
wasted.
Click again on 1.
After the registration, turn off the F12.
Click on 2
to save and 3 to quit the recording
screen..
The tension of the battery is shown in 4.
Don’t forget to change batteries of the ECG apparatus
when power is less than 7 Volt.
Also the LED on the F12 panel will glow red.
Use only batteries that could not leak !
Remark : every record occupies only 12 Kb hard disk
space for 20 seconds recording.

These settings are saved when you select OK.
NAME, FIRSTNAME, BIRTH DATE, ID, SEXE, AGE in years, LENGTH in cm and weight
in kg. These
settings can be altered.
FLOW
VOLUME LOOP :
Vertical flow in litre / second.
Horizontal volume in litre
On this figure the user can easily recognize
FVC Forced
Vital Capacity ( forced expiratory volume in litre) on the horizontal axis.
FEF
25 Forced Expiratory Flow ( litre / second ) when
25 percent of the volume is
expired.
FEF
50 idem when 50 percent of the volume is expired.
FEF
75 idem when
75 percent of the volume is expired.
PEF
Peak Expiratory flow maximal flow during expiration. ( Litre / minute )
This is what you can measure with an ordinary Peak Flow
meter.
PIF
Peak
Inspiratory flow, maximal flow during inspiration (Litre / minute ).
Remarks
PEF and PIF
are expressed in litre / minute, the
other values in litre / second.
FEF25 multiplied with 60 gives a value nearby the PEF
by most patients.
VOLUME
TIME
Vertical the expired volume in litre.
Horizontal the time in seconds.
On this figure it’s easy to read the ESW ( one second
value)
TIFF
(Tiffenau) is the proportion
between ESW and FVC.
Normal this value is 75 percent or more.
Lower ESW value : obstructive syndrome ( eg
asthma )
Global lower values : restrictive syndrome ( eg
Spondylitis ankylosans )
Leftmost column the abbreviation eg FVC.
Second column the measured values of the patient.
The third column the predicted value by means of sex,
age, length and weight.
The fourth and last column contains the percentage (
column 2 *100 divided by column 2 ).
If the measured value is larger or smaller than the
reference value of the patient plus of minus
two times the standard deviation, then it is marked
with an asterisk *.
PERSONAL
CONCLUSION :
Add your personal conclusion.
Medication, normal spirometrie, obstructive
spirometrie, restrictive spirometrie…
PRINT
A much higher resolution is performed with the A4
output mode.
The printing is independent of the screen’s
resolution.
Before the user can print, he is obligated to look at
the protocol.
If different printers are attached with Setup
- Printer select the printer for Spirometrie
printing.
Settings for the font, grid and signal are also
possible with Setup – Printer.
A few seconds after selecting the print option, the
user can continue to
work with windows.
Other
options of the header menu are :
Trace
- Delete
A confirmation is been asked.
Attention : Deleted is deleted
Quit
to stop
TRACE
export and import :
Export or import an ECG trace to A:\
This can be useful for the user with two PC’s.
Put a floppy disk in disc drive
A:\
From ECG32.EXE select Trace – export to export a trace to A:\
Put the floppy disk in another PC.
Start ECG32.EXE.
Select Trace -
import.
This option can also be useful when you work with two PC's.
Every time you look at a spirometrie, the values are
stored to an ASCII file, Spiro.txt
These can by captured by your software developer.
More
important is the following possibility for integration.
Starting from a patient program it’s very easy to
start de ECG software directly.
The advantage is that it’s not necessary for the user
to retype the name, first name,
birth date and ID of the patient.
Start the ecg32 program with parameters “name” “first name” , birth date,M or
F, ID,
Age, length and weight.
If the last 4 parameters are not present, you can still
enter them after selecting
Record. Name and first name can be in different parts and must
be putted between “
“
An single line of software code is enough.
In different programming languages it’s some thing
like
Winexec(‘C:\ECG32\ECG32.EXE
“SMITH” “JOHN JOE” 20/08/1951
M
123
52 176 71’,sw_shownormal);
The ECG32 program in the directory ECG32 is started and
jumps to
SMITH JOHN JOE If
this patient is not yet there, he will be added
after recording a new Spirometrie
( click immediately on the leftmost icon after starting the
program )
The formulas that are used by the software on the
calculations of
The reference values are
Age is in years and length is in cm.
For
Men :
FVC
= Length*5.77-Leeftijd*0.025-4.34
ESW =
Length*4.30-Leeftijd *0.029+1
TIFF
= -0.18*Leeftijd+87.2
FEF25 = Length*5.46-Leeftijd
*0.029-0.47
FEF50 =
Length*3.79-Leeftijd*0.031-0.25
FEF75 =
Length*2.61-Leeftijd*0.026-1.34
PEF
= (Length*6.14-Leeftijd*0.043+0.16)
* 60
For
Women :
FVC
= Length*4.44-Leeftijd*0.026-2.89
ESW =
Length*3.95-Leeftijd *0.025-2.60
TIFF
= -0.18*Leeftijd+87.2
FEF25 = Length*3.22-Leeftijd
*0.025+1.61
FEF50 =
Length*2.45-Leeftijd*0.025+1.15
FEF75 =
Length*2.61-Leeftijd*0.024+1.11
PEF
= (Length*5.51-Leeftijd*0.030-1.10)
* 60
The constant values multiplied with 1000 are in the
file C:\WINDOWS\SPI32.INI
Some user wants to use other values :
With a ASCII editor like NOTEPAD it is possible to
change this values.
First, take a backup of ECG32.INI
